Requesting medical records from St. Mary’s Healthcare is simple and convenient. Records may be requested online or in person.

Attorneys, insurance companies, and other authorized organizations

In person, Mail or Fax Requests

You may request records by submitting a completed Records Release Authorization Form by mail or fax. You may also send a signed and dated letter that includes:

• The records you are requesting
• Where the records should be sent
• A phone number where you can be reached

You may also contact the Health Information Management Department to:

• Request records or test results via secure email
• Sign up for the patient portal
• Arrange to pick up records
• Have records mailed to your home

Records sent directly to another healthcare provider for continuity of care are provided at no charge. For all other requests, St. Mary’s Healthcare follows New York State’s reasonable cost guidelines, which may include charges for supplies, labor and postage (if mailed.)
