COVID-19 ONLINE WAITLIST UPDATE: Due to the overwhelming amount of responses we have received and the shortage of COVID-19 vaccines across New York State we are unable to continue to accept more names for the vaccine waitlist until we receive further clarification from the State. Even if you already signed up for the St. Mary's COVID-19 vaccine waitlist, we do recommend that you keep looking for an appointment through other avenues (county offices, state clinics, local pharmacies) as they may receive more vaccines before we do. If we receive more vaccine allotment that allows us to vaccinate past our current waitlist we will open up this form again. Thank you for your patience and cooperation during this challenging pandemic.

St. Mary’s Healthcare has launched an online form for eligible New York State residents that captures all the information we need to add you to our COVID-19 waitlist when vaccines are available. Once vaccines are available for your position on our waitlist, a St. Mary's representative will contact you to set up an appointment. At this time, we are continuing to receive sporadic shipments of vaccines. Unfortunately, we are unable to predict when we will receive more shipments. We continue to advocate for our community to receive more vaccines, but we are subject to the NYS distribution process. Again, know that we will add you to our waitlist and will call when we have vaccines available, but we do recommend that you keep looking for an appointment through other avenues (county offices, state clinics, local pharmacies) as they may receive vaccines before we do.

We encourage everyone to use the online form for the quickest and most efficient way to be added to our wait list. If necessary, you may call (518) 842-1900 (press option 1) for anyone not comfortable with the online process or for anyone without internet.  

Please Note: If you have questions about whether or not the vaccine is safe/appropriate for you at this time, please contact your healthcare provider. This form or hotline will not be able to answer any specific medical questions.

**If you are concerned you may not be eligible, you can check your eligibility at: https://am-i-eligible.covid19vaccine.health.ny.gov/ and return to the St. Mary’s site to continue the online form.
