To ensure that patient wishes are known, respected and honored we encourage you to provide your primary care provider a copy of your directives which may include a Healthcare Proxy, Living Will, Non-Hospital DNR or Medical Orders for Life Sustaining Treatments (MOLST) Form. 

If you do not presently have these important documents St. Mary’s Healthcare offers patients and families the opportunity to access information and resources on how to discuss and make known your personal wishes about treatment decisions.  

For more information please you may inquire with your provider’s office about resources and forms to assist you or you may access and print these documents from home.
