St. Mary’s Healthcare Diabetes Prevention Program is an evidenced-based lifestyle change program recognized by the CDC. This year-long program includes 16 weekly sessions followed by monthly maintenance sessions for an additional 6 months. Sessions focus on healthy eating, physical activity and stress management. Participants will receive support from a trained Lifestyle Coach and are provided with the opportunity to bond with other participants who share similar goals.
The overall goal of this program is to help prevent or delay the development of type 2 diabetes by helping participants achieve a 5-7% weight loss and increase physical activity to 150 minutes per week.
You may be eligible for this program if you:
Are at least 18 years or older
Have a body mass index (BMI) of 25 or higher (23 or higher if Asian American)
Have not be previously diagnosed with type 1 or type 2 diabetes
Are not pregnant
Have been diagnosed with prediabetes, have a history of gestational diabetes or score a 5 or above on the prediabetes risk test
